Easily order collection kits for all US BioTek testing.
US BioTek offers a range of collection kits with all the supplies needed to collect a perfect specimen for all tests offered by US BioTek. Follow the steps below to order your collection kits and start seeing better clinical results with better testing.
1.) You Must Have An Account
US BioTek provides all of its kits free of charge, including shipping for practitioners in the US and Canada, to all providers that have a US BioTek Account.

2.) Kit Request Form
Once you have a US BioTek account you may request collection kits using our online Kit Request Form.

3.) Choose Your Kits & Quantities
From the Kit Request page, simply choose which kits you would like to order. The form has some guidance as to what each collection kit can be used for. You can also find the kits used for specific panels on the test pages under the right hand tab for Collection Instructions.
When you know what kit types you need simply choose the kit quantity from the drop down options.
US BioTek does reserve the right to adjust, to cap, or to decline orders based on unverified or delinquent account status. If this is to occur, a US BioTek representative will notify you of the changes.
4.) Enter Provider Information
Once you have selected the kits need for your order request, you must enter your provider information that is associated to your US BioTek Account.
It is important to make sure this information matches your US BioTek Account information to prevent fulfillment delays and order rejections.
5.) Submit Your Order Request
After filling our the kit order request form in its entirety, simply click submit!
Your request will be processed with the goal to ship all orders out within 24 hours of the order submission.
